AIIS will complete the First Report, provide copies to all interested parties, and make all state filings.

· Thorough Investigation
· Paying employee benefits on time
· Assigning lower case loads to claim adjusters
· Facilitating on-going communication with employee, employer and medical providers
When filing a First Report of Injury or Illness to your Insurance Carrier
· Complete First Report of Injury or Illness (LES form DWC-1). The employee should sign, if possible, and receive a copy of the First Report.
· Send First Report of Injury or Illness to your insurance carrier within seven (7) days of your knowledge of the accident or illness. Failure to report in time can result in a Division penalty of up to $1,000.
Note: All Deaths are to be reported to the Division of Safety by calling (800) 219-8953 within 24 hours.
